Job Title: LPN Team Case Assistant STATUS: Full Time LOCATION:
Rochester , NYDEPARTMENT: Home Care SCHEDULE: Days SUMMARYThe LPN
Team Case Assistant will u tilize clinical experience, and
coordinate patient services under the direction of the team
manager/designee. This includes but not limited to prioritization
and coordination of daily patient schedules, requesting additional
services such as aide, therapy, social work, interpreter services,
and ensuring that referrals for services are appropriately assigned

- Graduate of an accredited school of licensed practical nursing,
required
- A valid NYS license as a Licensed Practical Nurse
- Proficient use of Microsoft Office
- One year of experience in home care setting or medical office
preferred RESPONSIBILITIES
- Customer Service. Manages multiple phone calls in a timely and
professional manner providing appropriate follow up to assure
callers' needs are met. Validates supplemental orders and processes
per policy.
- Supports Field Staff. Provides field staff assistance and
resources for appropriate day to day issues as they arise under the
direction of the team manager/designee. Remains available to assist
in other duties deemed appropriate to the scope of practice of an
LPN.
- Program Review. Collects, organizes and reviews various
programs for information and reporting and initiates follow
up.
- Discharges. Performs functions related to transfers or
discharges as required by regulation.
- Clerical Work. Performs general office duties/projects
including but not limited answering phones, documentation/order
processing, copying, filing, mailing, and obtaining/ordering office
supplies.
- Orientation. Assists in orientation of new staff, role
modeling, participates in quality assurance activities.
- Required Qualifications:--- A valid NYS license as a Licensed
Practical Nurse.--- Valid NYS Driver's LicensePreferred
Qualifications:--- 1 year of nursing experienced preferred.---
Prior home health, clinical and direct patient care experience

BLS - Basic Life Support - American Heart Association (AHA), RN -
Registered Nurse - New York State Education Department
(NYSED)PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50
pounds of force occasionally, and/or 10 to 25 pounds of force
frequently, and/or greater than negligible up to 10 pounds of force
constantly to move objects; Requires frequent walking, standing or
squatting. For disease specific care programs refer to the program
specific requirements of the department for further specifications
on experience and educational expectations, including continuing

be considered for accommodations. PAY RANGE:$23.85 - $29.25 CITY:
Rochester POSTAL CODE: 14607 The listed base pay range is a good
faith representation of current potential base pay for a successful
full time applicant. It may be modified in the future and eligible
for additional pay components. Pay is determined by factors
including experience, relevant qualifications, specialty, internal
equity, location, and contracts. Rochester Regional Health is an
